Reservation Form

Please pay special attention to any media you are reserving or bringing in for viewing. You must have these materials reserved and cleared BEFORE you submit this form. We will not accept any reservation requests without confirmed media authorization and information, such as the Media Library or Film and Television Archive catalog number.

Once submitted, we will check our Reservation Calendar with the information provided to see if there are any schedule conflicts. We will then notify you by e-mail if there is a conflict. If the reservation request goes through, we will e-mail you a reservation number which confirms your request.
